Comprehensive Case Management

Our case managers for elders are more than a caregiver.  They are college degreed, where they focus on care management of the client at the masters level, along with overseeing "the total" needs of the client.

Unlike other caregiving companies, Sterling has a nurse practitioner / clinical psychologist on staff who can work with the primary physician and other professionals. Our case managers are required to have an emphasis in forms of dementia such as Alzheimer’s disease.   A clinical assesssment will begin the process of identifying the specific needs of your parent or family member.  We then customize an intentional care plan to meet the goals of the client/family.   

What you can expect with our case management team
  • Act as a liaison to families at a distance or locally, overseeing care, communicating with other professionals involved, (i.e., medical doctors, attorneys, dentist, nurses, etc.) and quickly alerting families to problems
  • Communicate with families about the client's status whether “doing well”, or potential concerns
  • Observe medications priscribed and compliance
  • Observe and report any suspicions of elder abuse: physical, emotional, financial, and sexual
  • Accompany the client to medical, dental, and/or other service appointments
  • Access best available resources in the community
  • Arrange for transportation to appointments, shopping and out of town travel
  • Ensure home safety, arrange for reconstruction of needs within the home, i.e., bathroom redesign
  • Arrange for various services for the client in their own setting, i.e.,  physical therapy, massage therapy, individual therapy
  • Supervision and coordination of in-home caregivers assigned to the client
  • Communicate with nurses, board and care owner’s, etc. about status of the client
  • Provide consumer education and advocacy.
  • Provide crisis intervention.

This means that we have more insight into personality and daily behaviors.
